It is a document jointly signed by the parties to a case stating the nature of the dispute and affirming the parties’ intention to arbitrate and abide by the award.
Contents of Submission Agreement. The Agreement shall contain, among others, the following: -
The parties’ intention to submit to arbitration; -
The specific issue/s or dispute/s to be arbitrated -
The name of the arbitrator or panel of arbitrators; -
The commitment to perform or abide by the decision or award. -
The arbitration cost

Subsidy Entitlement Form
It is a document signed by either or both parties to a voluntary arbitration case stating the intention of the parties to avail of the subsidy.
